Case managers provide assistance with completing various social service, educational, and financial applications. In addition, case managers assist clients with doctor visits, food bank needs, shelter, permanent housing assistance, social security assistance, rent or mortgage assistance, Medicaid, HEAP, SNAP, and more. 

Responsibilities:

  • Data entry and timely submission of case notes
  • Meet with several clients a day and assist in the necessary case management needed
  • Conducts initial assessment and establishes needs; conducts follow-up and timely reassessments
  • New client set up to process requests and obtain the necessary information needed for the case to be reviewed
  • Interact with clients to provide status updates on their requests and to ensure that they are processed in a timely fashion
  • Maintain a consistent schedule to meet with clients and write case notes efficiently
  • Referrals to other networks and agencies as necessary
  • Advocates for services that support self-sufficiency and self-management
  • Participate in monthly, quarterly, and annual staff meetings and trainings
  • Create monthly, quarterly, and annual reports discussing client outreach and caseloads
  • Community outreach and civic engagement
  • Other duties as assigned

Qualifications

  • Must be bilingual in Arabic/English
  • Some experience in case management strongly preferred
  • Outgoing, able to develop relationships, familiar with the human services landscape, and able to think innovatively and creatively.
  • Reliable transportation and a willingness to travel as needed
  • Proficiency in Microsoft Office Suite and Google Suite
  • Excellent organizational skills
  • Solid written and verbal communication skills
